Navigating the DDD Service System – 

In recognition that the process of exiting the School System and transitioning into the adult service  system can be overwhelming for students with intellectual and developmental disabilities (IDD) and  their families, the Division of Developmental Disabilities’ (DDD) Transition to Adult Services &  Employment Unit is available to provide education, resources, and guidance to school personnel,  students, families, and community organizations.  

The Transition to Adult Services & Employment Unit can provide the Navigating the DDD Service  System presentation, which provides an overview of the DDD service system, the necessary steps to  complete the eligibility process, the two programs available through its comprehensive waiver, and  information about the Division’s Care Management System - Support Coordination.
